Instructions for registration

Patient registration is a 2 step process. Please download and complete the age appropriate registration forms and also complete each of the Online Health and History Questionnaires.

College and Nall Office

• Option 1 – If you opted into our mobile registration feature, by providing us with your cell phone number and/or email address, a custom link from Phreesia will be sent to you via text and/or email with registration instructions.

• Option 2 – You may register in office. Please come in 20-30 minutes ahead of your scheduled appointment time.

South Overland Park Office and 150 Highway Office

Step 1: Patient Registration Forms

• Adult Registration: Adults and children 14 years of age and older

• Child Registration: Children 13 years of age and younger

 

Bring the completed forms to your appointment. Completion of the documents prior to your visit will speed up the registration process.
